It would seem some negative reinforcement is in order. Obviously you've considered diapers but spared her some embarassment by getting her Goodnights. You need to take away that little comfort by getting her some real diapers, Attends or Depends work well and make enough noise to alert passers-by of your daughter's diapered state. Insist on changing her in public areas as often as possible to maximize the humiliation factor. It's important you leave her an "out" by aggreeing to let her out of diapers once she can use the potty like "big girls" do. Let her pull the diapers down and use the toilet, if she wants. If she goes 3 days without needing to get her diaper changed, end the punishment. She'll have learned her lesson, believe me! Of course, if you don't like the idea of changing diapers there's the option to "reward" every wetting or pooping incident with an immediate spanking. I'd happily discuss it further if you wish.
